  5. Directors’ ID card (KTP)
  6. Company & Directors NPWP
  7. Statement of the person entitled to sign the letter for registration and licensing
  8. Packaging label concept
  9. Letter of proof of trademark registration/trademark certificate from the authorized agency
  10. Stamped statement letter from the applicant that the required documents are complete and correct
  11. Fertilizer Description
  12. Proof of PNBP Payment
  13. Power of attorney from the Formula Owner from abroad to the business entity
  14. Certificate and/or Quality Test Results Report (LHP)
  15. SPPT-SNI for fertilizer must be SNI and
  16. Effectiveness test results report
