Mutation of Address/Domicile

Requirements:

1. Application form for Change of Address;
2. Application for change of address;
3. Sponsor’s KTP (photocopy);
4. Guarantee Statement from Sponsor;
5. Certificate of domicile from the apartment if the foreigner lives in the apartment; If you live in housing, a domicile certificate from the Kelurahan Office.
6. Power of Attorney (if authorized);
7. Foreign passport (original and photocopy)
8. KTP authorized (photocopy);
9. KITAS Online (photocopy);
10. For foreigners holding KITAP, the original and a copy of the KITAP Card must be attached;
11. For foreign workers, attach a work permit;
12. For the sponsor’s wife/husband, the Indonesian citizen attaches a Marriage Book, Sponsor’s KTP, and Sponsor’s Family Card (photocopy);
13. For Foreign Investors (PMA) attach a Recommendation from the Investment Coordinating Board (BKPM) (copy);
14. For students, attach a letter of recommendation from the relevant agency.

Transfer Sponsor

Requirements:

1. Application letter from the new sponsor;
2. Statement and guarantee from the new sponsor;
3. Sponsor’s KTP;
4. Original passport and photocopy;
5. A statement from the old sponsor (which states that does not mind switching sponsors)
6. Original KITAS or KITAP and photocopy;
7. For the sponsor of the wife or husband of an Indonesian citizen, attach a Marriage Book, KTP and Family Card;
8. For sponsors of Indonesian citizens’ parents, attach the applicant’s birth certificate with a certified Indonesian or English translation;
9. Domicile from the kelurahan office;
10. For foreign workers, attach the latest Work Permit as well as company documents;
11. Attach TA03 from the Ministry of Manpower.

Passport Mutation

Requirements:

1. Passport Transfer Application Form;
2. Application for Mutation of Passport;
3. Sponsor’s  KTP (photocopy);
4. Power of Attorney (if authorized);
5. KTP authorized (photocopy);
6. Old and new foreign passports (original and photocopy)
7. KITAS Online (copy);
8. For foreigners holding KITAP, the original and a copy of the KITAP Card must be attached;
9. For foreign workers, attach Work Permit, etc. (photocopy)
10. For the sponsor’s Wife/Husband, Indonesian Citizens attach a Marriage Book, Sponsor’s KTP, and Sponsor’s Family Card (photocopy);

9. For sponsors of Indonesian citizens, Indonesian KTP and Indonesian Family Cards (photocopy)
10. Child’s Birth Certificate (original from Dukcapil and photocopy);
11. Attach a Birth Certificate from the Hospital (photocopy).

Double Position

Requirements:

1. Application letter from sponsor;
2. Statement and guarantee from the sponsor;
3. Sponsor’s KTP
4. Original passport and photocopy;
5. Original KITAS and photocopy;
6. Domicile of the kelurahan;
7. Attach the latest Work Permit and the original/previous original and photocopy and company documents;
